From Janitor to Healthcare Leader: The Journey of Michael Dowling

Michael Dowling, the retiring CEO of Northwell Health, shared his remarkable story of rising from humble beginnings to leading a vast healthcare network comprising 28 hospitals and over a thousand outpatient centers across the tri-state area.

Growing up in Ireland in a thatched-roof house without running water, Dowling moved to New York at age 16 in the 1960s with hopes of a better life. He worked various manual jobs—on boats, in construction, plumbing, and cleaning bars—to support himself. These early experiences instilled a strong work ethic and resilience that he credits for his later successes.

Later, Dowling pursued higher education, earning an undergraduate arts degree and a master’s in human-services policy from Fordham University in 1974. He began teaching social policy at Fordham and rose to assistant dean. His expertise led to roles at the Department of Health and Human Services before he was recruited in 1995 to North Shore University Hospital on Long Island.

In the mid-1990s, Dowling played a critical role in merging North Shore with the struggling Glen Cove Hospital, initiating a chain reaction that transformed healthcare services across Long Island. Despite opposition—particularly from the Justice Department, which initially sued to block the merger—his team prevailed after a court trial. These acquisitions laid the foundation for Northwell’s expansion into New York City, Westchester, and, recently, Connecticut, with a merger with Nuvance Health. Dowling now envisions extending its reach into New Jersey.

Beyond strategic expansions, Dowling emphasizes the importance of staff engagement. He actively interacts with thousands of employees through dinners, daily hospital floor visits during the pandemic, and weekly orientation sessions. Known for his candidness and leadership, he advocates a no-nonsense attitude, urging staff to “suck it up” and focus on opportunity rather than challenges.

Dowling’s story exemplifies perseverance, emphasizing that success often involves hard work and resilience. As he steps down, he remains committed to fostering leadership and embracing opportunities for growth within the healthcare sector.
